Shegan Mahuang Tang is one of the classic formulas of traditional Chinese medicine, first recorded in the Jinkui Yaolue by the Eastern Han physician Zhang Zhongjing, and it is a commonly used formula for treating cough and wheezing disorders. The whole formula is composed of Shegan (blackberry lily rhizome), Mahuang (ephedra), Shengjiang (ginger), Xixin (asarum), Ziwan (aster), Kuandonghua (coltsfoot flower), Banxia (pinellia), Wuweizi (schisandra) and Dazao (jujube), and through careful combination these herbs together exert the effects of diffusing the lung and stopping cough, and transforming phlegm and calming wheezing.

In terms of effects and indications, Shegan Mahuang Tang mainly diffuses the lung and dissipates cold, and directs qi downward and transforms phlegm, and traditionally it is mostly used for cough and wheezing caused by cold-fluid stagnation in the lung. This pattern often presents as cough and panting, phlegm rattle in the throat like the sound of a water fowl, and copious thin clear phlegm, with symptoms often worsening after exposure to cold. Because its medicinal nature is warm, it is more suitable for patients with cold-type asthma and cold-phlegm cough.

In terms of combination characteristics, Mahuang in the formula diffuses the lung and calms wheezing, Shegan eliminates phlegm and benefits the throat, and the pairing of these two is the core of this formula; Xixin and Shengjiang warm the lung and dissipate cold, Ziwan and Kuandonghua stop cough and transform phlegm, Banxia directs qi downward and eliminates phlegm, Wuweizi astringes lung qi to prevent excessive acrid dispersion, and Dazao harmonizes the middle and protects the stomach. The whole formula contains astringing within dispersing, and eliminates pathogenic factors without harming healthy qi, reflecting the medication approach of one astringing and one dispersing, one opening and one closing.

It should be noted that Shegan Mahuang Tang is a prescription medication, and herbs such as Xixin in the formula have a certain toxicity, so the specific dosage and modifications must be determined by a traditional Chinese medicine physician based on syndrome differentiation, and one must not gather and take the herbs on one's own. If heat signs such as yellow and sticky cough and wheezing phlegm, fever and thirst are obvious, this formula is generally not suitable, and one should seek medical attention promptly to clarify the syndrome pattern before treating according to the symptoms.
